*A deposit of 20% is required for all appointments, and 25% for FMTY.

*All cancelations within 48 hours of scheduled appointment are subject to a 50% fee.

*All cancelations within 24 hours of scheduled appointment are subject to a 100% fee.

*All FMTY/travel cancelations within 72 hours are subject to a 50% fee & within 48 hours are subject to a 100% fee. Rescheduling to an agreed upon date will allow entire rate paid to be pushed to future date.
